Our application/contract is available in Microsoft Word and Adobe Reader. 

Please download the form, fill it out and Express Mail it with your application fee(s) and a copy of your photo ID to Heavenly Meadows Horse Rescue, 4895 Grassy Pond Rd., Chipley, FL 32428.

Note: You can fax us your application at 850-773-9991, however, if you fax us the application we must receive your application fee and original application within 48 hours (or at least your tracking information) or we will not proceed with processing your application.

If you do not have Microsoft Word or Adobe Reader please e-mail us (info@heavenlymeadows.org) and we will send you an application via e-mail or fax.

IMPORTANT: Read through the application carefully and make sure you sign and initial EVERY spot you need to. We do not want to deny your application because of a missed signature or initial. And do not forget to add a copy of a photo ID!
