THIS IS ONLY FOR HORSES THAT ARE LOCATED AT HMHR AND ADOPTED OUT BY HMHR. FOR ANY OTHER HORSE LISTED ON OUR SITE YOU MUST CONTACT THE PERSON LISTED BY THAT HORSE!!

 

Our application/contract is available in Microsoft Word and Adobe Reader. 

Please download the form, fill it out and Express Mail it with your application fee(s) and a copy of your photo ID to Heavenly Meadows Horse Rescue, 4895 Grassy Pond Rd., Chipley, FL 32428.

 

Again, this is only for horses that are here at Heavenly Meadows. For adoption of other horses listed on our site you MUST contact the person listed next to that horse.

If you do not have Microsoft Word or Adobe Reader please e-mail us at info@heavenlymeadows.org and we will send you an application via e-mail.

IMPORTANT: Read through the application carefully and make sure you sign and initial EVERY spot you need to. We do not want to deny your application because of a missed signature or initial. And do not forget to add a copy of a photo ID!
